Required Reports & Due Dates

• Fire Incident Report – Due within 14 days of the occurrence

Report Submission

Submit reports to your local Fire Safety Officer (FSO) or ANA Fire Protective Services Consultant.  These officers will review the report and submit the information to OFC.
FSOs, ANA Fire Protective Services Consultants, and fire departments can directly submit fire reports to OFC by Mail, Fax or Email. (Contact Information)
